By filling out information about my child I agree to the conditions specified below:
While participating in the Monte Vista Institute sponsored by the Debate Team at Monte Vista High School, my child agrees to conduct him or herself in an appropriate manner and to respect the others who are participating in this camp.
In the event of an accident, damage or injury occurs, I shall defend, indemnify and hold the Monte Vista Institute, Monte Vista High School, its officers, employees and agents harmless from and against any and all liability, loss, expenses, (including reasonable attorney’s fees) or claims for injury, loss or damages arising out of my participation in this activity but only in proportion to and to the extent such liability, loss, expenses, attorney’s fees or claims for injury or damages are caused by or result from the negligent or international acts or omissions of applicants, its officers, agents, employees or members.
I give the Monte Vista Speech and Debate Institute permission to use my child’s photographic likeness in all forms and media for advertising, trade, and any other lawful purposes.
I understand that the Monte Vista Institute and Monte Vista High School, its officers, employees and agents will not be responsible for any loss, damage, or destruction of my personal property.
PERMISSION FOR EMERGENCY TREATMENT:
I understand that David Matley or another Institute staff member will attempt to contact the parent, guardian or person listed on this emergency form in case of illness or injury.
I authorize David Matley or another Institute staff member to contact paramedics when emergency medical attention is warranted. I give permission for my child to be taken to a hospital and given medical attention by hospital staff as necessary. I understand that I am responsible for any charges incurred.
